Accident

An unplanned event or sequence of events that results in undesirable consequences. An incident with specific safety consequences or impacts. The term accident is defined as any incident which is accompanied by actual negative consequences.

An unwanted occurrence which results in injury to a person, or loss or damage to equipment, materials or the environment.
