Technique for human error rate prediction (THERP)

Technique for human error rate prediction. The probability of human error is assumed to be function of a series of "performance shaping factors" such as temperature, hours worked, complexity, availability of tools, ? (about 70 in total). It includes the likelihood of detection, the probability of recovery and the consequence of the error (if uncorrected).
